Tots 'n' Teens Pediatrics offers consultation visits to address the concerns of our patients and families who may be experiencing behavioral, educational, or mental health issues that require lengthy consultation and discussion with the provider.
If a consultation visit is needed to discuss school problems with possible attention and/or hyperactivity concerns, please print out the Vanderbilt Questionnaires for parents and teachers. Your provider will need at least one teacher form completed and at least one parent form completed, but we welcome forms from any educators or caregivers who have frequent and ongoing contact with your child. Please fax completed forms to 205-856-4445. Once your provider has reviewed the forms, our office will reach out to set up a consultation appointment.
